Some patients reportsinus infections during the hayfeverseason, and it is commonly supposed thatcongestion associated with allergic rhinitiscan predispose to infectious sinusitis byinterfering with the function of the sinusostia. However, there is no good evidence tosupport this hypothesis, and no good evi-dence that specific treatment by allergen avoidance or immuno-therapy (or even non-specific treatment with continuous topicalcorticosteroids) can prevent acute infectious sinusitis in peoplewho do not have underlying chronic sinusitis. Most episodes ofacute sinusitis are sequelae to viral upper respiratory tract infec-tions. However, topical corticosteroids are an effective adjunct toantibiotic therapy in cases of acute infectious sinusitis, with orwithout coexisting allergy (Level II).
