Synopsis There are many observational studies, but no reported randomised trials, examining clinical endpoints (death or myocardial infarction) in primary prevention of cardiovascular disease with the use of HRT. The only randomised secondary prevention trial published did not demonstrate a reduction in cardiovascular events with HRT. On present evidence HRT should not be initiated for primary or secondary prevention of cardiovascular disease. Potential risks, such as breast cancer, must be considered against potential benefits. Most of the evidence currently comes from observational data, and the only randomised trial based on clinical events as endpoints indicates a need for caution in this area
