I assessed severe problems which occurred in 116 patients after cholecystectomy, and identified a wide variety of causes. I found organic disease in the biliary tree or pancreas in 88 patients, and confirmed this disease by endoscopic retrograde cholangiopancreatography (ERCP) in 79. I found stones, the most common single cause, in only 26 patients. We recommend ERCP as the procedure of first choice in the investigation of severe post‐cholecystectomy problems.
