One hundred and fourteen children suspected of having congenital rubella were tested for rubella‐specific immunoglobulin M (RSIgM) in the neonatal period by means of the sucrose density gradient ultracentrifugation method. Follow‐up information was obtained on 71 of these children, and showed that 41 of them had congenital abnormalities. Thirty‐seven of the 41 had clinical signs of congenital rubella and a positive RSIgM test results; four had negative test results. Two of the latter children undoubtedly have congenital rubella, but the diagnosis remains in doubt in the other two. Another 10 children with positive test results have so far shown no signs of congenital rubella, but may do so at later follow‐up. Many of the infants also had a total IgM estimation, and a good correlation was shown between raised total IgM levels and the presence of RSIgM and between normal total IgM levels and absence of RSIgM.
