All new students are required by the State of Connecticut to submit proof of vaccinations in advance of their arrival at Yale University. You should print the vaccination form and the instructions located on the Yale Health website (Undergraduate Students click here [1]and Graduate & Professional Students click here [2]) and take both documents to your doctor to have it completed. On June 20th, you can log into yale.medicatconnect.com [3] to upload your vaccination from. All vaccination information must be uploaded by August 1st.
