Parenteral hyperalimentation has been used on 57 patients at the Royal Prince Alfred Hospital for periods of up to 54 days. Our experience currently amounts to a total of 1,002 patient days. The technique used involves the infusion of a solution of synthetic amino acids, hypertonic glucose and other essential nutrients into the superior vena cava. This allows prolonged total rest of the gastrointestinal tract while maintaining adequate nutrition. Thus, an anabolic response is achieved under circumstances usually associated with a catabolic state. Recovery from catastrophic clinical situations, which were previously thought to be irreversible, has been accomplished. The technique has been particularly useful as an aid in the management of gastrointestinal fistulæ, spontaneous closure of such fistulas having occurred in 18 patients.
