The information, and the pathophysiological basis of this information, which should be obtained by investigating a patient with vesicoureteral reflux by intravenous pyelography (IVP), micturating cystourethrogram (MCU), and cystoscopy is reviewed. A protocol is suggested for the management of reflux based on this information. The aim of the protocol is to allow of surgical intervention in those cases at risk before upper tract damage has occurred. It is also hoped to avoid too heavy an emphasis on surgery in those cases which do not require operative intervention. The writer's experience with this protocol is also reviewed.
