The Tolosa–Hunt syndrome consists of a steady boring pain in one orbital region with ipsilateral oculomotor and pupillary sympathetic impairment, lasting for days or weeks and sometimes recurring after long remissions. Angiographic and operative findings on a few cases provided evidence of an isolated periarteritic disease of the intracavernous carotid artery or, in one patient, of a sphenoidal fissurltls. The present case offers some peculiar features not hitherto emphasized in the descriptions of the syndrome.
