The intestinal distension of retroperitoneal hsematoma is felt to be quite distinct from the usual causes of intestinal distension such as organic intestinal obstruction, paralytic ileus and adynamic ileus. It is suggested that in retroperitoneal haematoma there is usually a greatly increased formation of intestinal gas and greater motility of the bowel, but no obstruction to the lumen of the bowel. Four cases are described and possible mechanisms of this condition are discussed and the concept of vascular compression elaborated as a cause of the syndrome.
