Optimal histopathological interpretation of liver biopsies in public hospitals requires adequate clinical details which are often lacking. A “liver biopsy request form” is described which is simple to fill in. It contains enquiries concerning previous liver biopsy, reason for biopsy being done, relevant clinical and laboratory data, and exposure to sensitizing drugs. Its use should help in diagnosis and have educational value.
