Three cases of severe hæmolytic anæmia complicating infectious mononucleosis are described. The present experience concurs with previous reports that, despite its initial severity, the haemolytic process Is usually self‐limiting, although corticosteroids, as in two of these cases, or, rarely, splenectomy, may be indicated. In two of the cases, the primary diagnosis of infectious mononucleosis was difficult because of the atypical presentation as haemolytic anaemia, and the delayed rise of the heterophil antibody titre. It is suggested that in cases of unexplained haemolytic anaemia, repeated serological investigations may be necessary to exclude infectious mononucleosis as the primary diagnosis.
