Eighty‐two United States servicemen on leave from Vietnam were admitted to hospital with a diagnosis of falciparum malaria. The severity of the infection varied from a mild febrile illness to a severe life‐threatening illness. Seven of the 82 patients developed pernicious manifestations of falciparum malaria. Thrombocytopenia was a common finding at examination, as was lymphopenia, the presence of abnormal mononuclear cells in the blood smear, and a mild disturbance of liver function. The condition of 10 of the 14 patients given chloroquine had not improved or had become worse 2 to 8 eight days after the start of treatment with chloroquine. All but one of the 82 patients were regarded as having chloroquine‐resistant falciparum malaria and were treated with quinine, Daraprim and dapsone. All the patients recovered.
