RENAL FAILURE AND HYPERBILIRUBINÆMIA IN LEPTOSPIROSIS TREATMENT WITH EXCHANGE TRANSFUSION

Renal failure with total anuria and hyperbili‐rubinæmia were observed in a patient with leptospirosis. Peritoneal dialysis decreased nitrogen retention but failed to reduce hyperbilirubinæmia and improve the clinical condition. Exchange transfusions resulted in an effective decrease in the serum bilirubin level, which was followed by clinical improvement and diuresis. It is thought that hyperbilirubinæmia might compromise the renal function already impaired in leptospirosis. Exchange transfusion appears to be a useful procedure in addition to dialysis in the management of renal failure in leptospirosis associated with hyperbilirubinæmia.
