Vagotomy with pyloroplasty is widely employed for the treatment of duodenal ulcer. Recent reports have questioned this choice of operation. This review presents the evidence available from controlled trials of different operations, and concludes that vagotomy with drainage is a safer procedure than gastrectomy, but is associated with an unacceptable recurrence rate. Methods of reducing this recurrence rate are discussed.
