His bundle electrograms have been recorded in over 50 patients. A special electrode catheter is positioned across the tricuspid valve to allow clear recordings of a sharp His bundle (H) electrical spike. Defects in AV nodal conduction are manifest by a prolonged interval from atrial activity to H spike (AH interval) and defects in the peripheral conducting system by a prolonged H spike to ventricular activity time (HV interval). Examples are presented to show the potential value of this technique in the assessment of intracardiac conduction defects, of complex cardiac arrhythmias and in the further elucidation of sites of action of cardiac drugs.
