Granulomas presenting as reddish‐purple ovoid masses within the area of napkin contact on the perineum, thighs and scrotum of an infant aged 9 months are recorded. This condition was first described and named by Tappeiner and Pfleger who reported a series of cases in July, 1971. No treatment is required as these lesions resolve spontaneously. In the case of this patient, the mother had used zinc and starch powder in the napkin area and starch granules were identified in the histological section. Starch granules have not been observed in the lesions in previously reported cases.
