This is the first of two proposed papers based on the experience of treating 35 people with multiple major trauma, the result of motor vehicle collisions, during the past 3 years in a large rural centre. The purpose of Part 1 is to set out the form of management developed by the medical staff of the Cowra District Hospital. Part 2 will deal with roadside management and the establishment of a casualty emergency system. In this paper the emergency treatment within the hospital is discussed in detail. Resuscitation is described as the important immediate measure, followed by the emergency treatment of head and chest wounds. The early management is then discussed with emphasis on the treatment of chest wounds. Further general management is described with particular attention given to monitoring. In conclusion, some of the factors which could lead to an increase in the salvageable proportion of road accident victims are outlined.
