The value of secretin stimulation of pancreatic function was statistically evaluated in 68 patients and 15 control subjects. Seventy milliequivalents per litre appeared to be the lower limit of normal bicarbonate concentration after the intravenous injection of two units per kilogram of body weight of secretin (Boots). There was a general pattern of low volume and low bicarbonate concentration in chronic pancreatitis, but a proportion of patients with proven pancreatitis exhibited bicarbonate concentrations within the normal range. Low volume and a normal bicarbonate concentration suggested pancreatic neoplasm, but volume was found to be an unreliable parameter. A low bicarbonate concentration was found in two patients with mesenteric vascular insufficiency. Cytological examination of duodenal juice proved disappointing in the small number of patients with pancreatic cancer.
