One hundred patients consecutively admitted to a coronary care ward with a confirmed myocardial infarction were questioned concerning prodromata which may have occurred before their admission. Pain was found to be the most common prodromal symptom, and this occurred in 72% of patients. Pain was characterized as either an increase in the frequency of anginal attacks (32%), or the development of angina for the first time (40%). The development of angina for the first time or a change in anginal pattern should be regarded as a prodrome of acute myocardial infarction and the patient treated accordingly.
