The clinical features of 11 patients with mycotic aneurysms are described. The aneurysms involved a variety of vessels, some of which were relatively inaccessible to the surgeon. Six of the aneurysms were able to be excised with survival in five cases. The remaining five patients, on whom operation was not carried out, died. Mycotic aneurysms often present diagnostic clues suggesting their infective nature, but in many cases a presumptive diagnosis has to be made from the nature of the aneurysm without positive microbiological test results. Early diagnosis and adequate antibiotic therapy combined with prompt surgical intervention are important factors in management. The aneurysm should ideally be excised or excluded and if necessary the distal circulation restored by arterial reconstruction through a different route, as direct graft replacement may result in reinfection.
