A review is presented of reports in the literature on treatment of warts by intralesional inoculation with smallpox vaccine, and on complications following this procedure as well as those following ordinary vaccinations. The writer describes his experience in the treatment of 40 people with verrucæ vulgares by lesional acupuncture through topically applied smallpox vaccine. In the overall, an 80% cure rate has been obtained with generally minor secondary reactions. On the basis of these, he does not recommend smallpox vaccine treatment of warts as a routine procedure, but does feel that this form of treatment could be considered and employed for very resistant lesions, particularly multiple ones, and those situated in periungual situations, especially if accompanied by attenuation of the smallpox vaccine by heating to 56ºC for 20 minutes and the exhibition of methisazone or antivaccinial gamma globulin as indicated, preferably the former.
