In a subject presenting for examination for assessment of “dust” disease and respiratory incapacity, the non‐respiratory components, If present, having been first estimated, an attempt is made to analyse the respiratory abnormality into its three common components in the age group concerned, namely, pneumoconiosis, emphysema and chronic bronchitis. Occasionally other components such as tuberculosis, asthma, bronchiectasis, neoplasm, sarcoidosis, etc., are found. From studies of large numbers of post‐mortem lung specimens of coal miners, it has been possible to establish clinical, radiological, fluoroscopic and electrocardiographic guidelines which, together with the forced expiratory volume in one second and vital capacity readings, and the results of more sophisticated respiratory function testing If available, enable reasonable assessment of each of the above‐ mentioned components In a subject undergoing medical examinations. Guidelines are presented, together with an outline of certain principles used to decide attribution to each component, some of thess principles being arbitrary.
