This paper covers the historical background, the development, the present status and the possible future of cardiac surgery. It is based on experiences in the cardiac unit at St Vincent's Hospital. The surgery of the mitral valve is used to illustrate many basic problems. The paper is divided into the antenatal period ending with the birth of the speciality in 1949, the period of blind intracardial procedures (1950‐1955), the period of open cardiac surgery under hypothermia (1955‐1960), the period of open cardiac surgery under cardio‐pulmonary bypass (1960‐1965), and the period of maturity from 1965 to the present. The future is considered in the light of recent developments in the surgery of coronary artery disease. Problems in originating a service, in conducting a unit, and in the evolution of the first‐ generation cardiac surgeons are briefly outlined.
