The results are presented of surgical treatment of 41 patients with renal hypertension. Arterial reconstruction was preferred to nephrectomy in a desire to preserve functioning renal tissue. In almost two‐thirds of the patients treated by reconstruction, nephrectomy was contraindicated because of the extent of the disease. It was in this group that difficulties were encountered, and treatment was unsuccessful in about one‐third of the patients. In the remaining patients treated by reconstruction or by nephrectomy, failure of response was rare. Subsequently the condition of about one‐half of these patients was more easily controlled with medical treatment, while the other half were able to discontinue hypotensive drug treatment altogether. The best results occurred in young patients with fibromuscular hyperplasia.
