The surgical anatomy of Spigelian hernia is briefly described. X‐ray examination with a barium enema can be conclusive in the preoperative diagnosis, when the condition is clinically unsuspected or difficult to recognize in the obese patient. A case is presented in which the classical features were overlooked, through lack of awareness of this condition.
