Acute hypersensitivity to nitrofurantoin was manifested in a female patient by fever, asthma and pulmonary oedema. An accelerated reaction occurred a week later, one hour after a single tablet had been Ingested. Reports of serious nitrofurantoin reactions to the Australian Drug Evaluation Committee are discussed, and the literature is reviewed. The clinical spectrum includes subacute reactions and chronic pulmonary fibrosis. In acute cases often asthma is absent, but most of these patients develop basal infiltration shown by X‐ray films of the chest, and delayed eosinophilia. The differential diagnosis from chest infections and cardio‐vascular diseases, and the pathogenesis relating to pulmonary eosinophilic infiltration and œedema, are discussed. Treatment by adrenaline and corticosteroids is assessed.
