Although the majority of patients with ulcerative colitis can be managed by conservative measures, there are certain groups of patients whose life is at “high risk” from this disease. These “high‐risk” groups Include patients with severe attacks, those with extensive Involvement of the colon and rectum, and those who are aged over 60 years. Arguments are presented which may justify the following: 1. Earlier surgical intervention in patients with severe attacks. 2. Prophylactic colectomy for all patients with extensive colonic and rectal involvement, because of the risk of severe attacks and of the development of cancer. 3. A more radical approach to all patients aged over 60 years whose disease extends beyond the rectum into the colon.
