The problems that may be encountered in establishing the cause of a painful thyroid are discussed and illustrated by the clinical and laboratory findings for patients presenting with this relatively uncommon complaint. In particular, attention is drawn to the variability in the clinical picture of subacute thyroiditis and to the similar features that can arise in other thyroid disorders. Laboratory investigations can be helpful in identifying this condition, but no single procedure is infallible. The diagnosis can be definitely made only when the history and clinical examination are considered along with the laboratory findings, in particular the 131 I uptake, triiodothyronine resin uptake and ESR. The importance of establishing the diagnosis of subacute thyroiditis is emphasized.
