A report is presented of a case of acromegaly with severe prognathism. The patient suffered a severe facial injury, including a fracture of the malar‐maxillary complex with a blow‐out fracture of the floor of the orbit. A bone graft was needed to reconstruct the orbital floor. A cosmetic reduction of the chin prominence was carried out, and some of the bone was used for the orbital reconstruction. This corrected the displacement of the globe and relieved the acromegaly.
