In discussing cerebrovascular disease, some pathophysiological aspects underlying clinical problems are outlined. Only as these are understood more fully can we hope to improve our therapeutic approach and results. Aspects of management are considered at three stages: in the stage of transient ischæmic attacks, in that of established stroke, and in that of recovery from established stroke. Attention to transient ischæmic attacks is of paramount importance for the prevention of permanent lesions, the treatment of which is made difficult by our frequent uncertainty of the underlying pathological process. Methods of preventing further episodes of cerebral damage are still in the stage of evaluation.
