A description is given of a female aged 17 years who developed a unilateral lung obstruction 14 days after tracheostomy. Bronchoscopic extraction showed that the lesion was a bronchial cast formed by thrombosis of inhaled blood. The presence of this type of lesion should be considered when bronchial obstruction complicates tracheostomy management.
