1. A review is presented of the clinical pattern and the immediate results of treatment of perforated peptic ulcer at St Vincent's Hospital, Sydney, from 1954 to 1966. An unusually high incidence of females is noted, and the possible relationship of this feature to aspirin ingestion is discussed. 2. The long‐term results in 142 patients treated by simple suture of the perforation are presented. 3. On the basis of these results, it is suggested that an immediate definitive operation is the optimum treatment for perforation, except for pyloro‐duodenal ulcers with a short history.
