A case of cardiac arrest during the operation of prostatectomy is described. Two hundred and twenty‐one defibrillations were subsequently performed, with final reversion to sinus rhythm after the administration of propranolol. The patient recovered well from the cardiac arrest and fibrillations but died three months later of complications resulting from a tracheo‐œsophageal fistula. The literature on tracheo‐œsophageal fistula following intermittent positive pressure respiration through a tracheostomy tube is reviewed.
