Dissecting aneurysm of the aorta can be divided into three types, depending upon the extent of the dissection and the diameter of the aortic lumen. Cystic medial necrosis, hypertension and trauma are predisposing factors. The prognosis without surgery is poor, death being due to the distal effects of dissection or proximal aortic incompetence, tamponade or coronary artery damage. A few cases present in a subacute or chronic form. Surgical management will depend on the anatomy encountered, and whether or not resuspension of the commissures, aortic‐valve replacement or replacement of the ascending aorta, or a combination of these, is indicated. A series of seven cases is described.
