Acute renal failure following intravenous cholangiography with “Biligrafin” is described in a patient with Dubin‐Johnson syndrome. This association has apparently not been described previously. Acute tubular necrosis was probably produced by a direct toxic effect of “Biligrafin”. Increased renal excretion of “Biligrafin” because of impaired hepatic excretion of the drug is advanced as a contributory factor in the production of the renal lesion. Care is advocated in the management of patients with cholestatic liver disease with regard to the use of intravascular contrast media normally excreted by the liver.
