Two cases of immediate rejection of a renal allograft in the presence of a negative cross match between recipient serum and donor leukocytes are reported in detail, together with the relevant immunological and histological investigations. One case is considered to have been caused by a histocompatibility antigen‐antibody reaction, with a false negative cross match. The second case is considered to represent a Shwartzman reaction in the presence of a true negative cross match. A negative cross match, in its present form, does not exclude entirely the possibility of the occurrence of an immediate rejection of a renal allograft.
