A case is presented of concomitant hyperparathyroidism and acute pancreatitis. The serum calcium level fell from near crisis levels during treatment with sodium phosphate, and the patient subsequently underwent parathyroidectomy under prophylactic cover with “Trasylol”. Two large parathyroid tumours were removed, after which the serum calcium level remained normal for some months, but is now fluctuating at the upper end of the normal range. The literature is reviewed, and it is concluded that neck exploration in the absence of frank hyperparathyroid crisis can usually be postponed until the patient's general and pancreatic condition has become stabilized. Long‐standing hypercalcæmia producing nephrocalcinosis and chronic renal failure may complicate successful surgical treatment of hyperparathyroidism.
