Sarcoidosis is a systemic inflammatory condition that primarilyaffects the lungs, lymphatic system and skin. Extra thoracic manifestationsoccur in about 50% of cases; however, it rarely affectsthe male reproductive tract. The first documented case of epididymaland testicular sarcoid in the Irish literature is of a 25-year-oldmale who presented with a painful 1-cm extra-testicular scrotalmass. While the involvement of the male reproductive tract israre, it should be included in the differential of testicular pathology.It can be diagnosed on frozen section and may be managedconservatively.
