Langerhans cell histiocytosis is a common tumor in the second decade of life. Distal humerus metaphysis is an uncommon site of Langerhans cell histiocytosis. We report a case of distal humerus solitary eosinophilic granuloma in a 13-year-old boy with fever and intraoperative findings of granulation tissue and pus. Biopsy provided a definitive diagnosis. As it was benign, the lesion was treated conservatively. Two years postoperatively, the patient has good elbow range of motion with no recurrence. [Hand Microsurg 2017; 6(1.000): 47-50
