A neonatal case of type 1 pseudohypoaldosteronism is described. The patient showed difficulty in sucking associated with hyponatremia, hyperkalemia and elevated plasma renin and aldosterone levels. ACTH and 17-OH-progesteron plasma levels were normal. Sodium chloride supplementation was effective in controlling symptoms and normalizing plasma biochemical alterations and it was no longer necessary after the sixth month.
