Overview: Spontaneous pneumothorax with contralateral air cyst is common in clinical practice. The most common treatment today is single-stage bilateral bullectomy by transthoracic VATS. Recently, bilateral bullectomy by subxiphoid uniport VATS is a new step. There are not many reports on this topic in the world, in Vietnam, there is no report. Method: Based on a sergery at Pham Ngoc Thach hospital and reviewing the literature, the report aims to evaluate the performance, advantages, disadvantages of the surgery. Result: male patient, 53 years old, spontaneous recurrent right pneumothorax, bilateral bullous lung disease on CT Scan, simultaneous bilateral bullectomy surgery by subxiphoid uniportal VATS. Conclusion: Subxiphoid uniportal VATS bilateral bullectomy can be performed successfully provided that the surgical team is experienced in uniport VATS.
