A 28-year-old male patient admitted to our clinic with complaint of deformity and pain in his back, we detected significantly advanced kyphosis deformity at physical examination. His increasing deformity was known since childhood ages and never treated elsewhere.126-degrees of kyphotic deformity was detected at radiological examination and measurements. (Figure 1) He complaints about having difficulty of breathing and continious pain. Posterior instrumentation and fusion was recommended. Before surgery patient informed about the risks of the surgical procedure and he did not accept treatment. In this case we can observe the progression potential of the kyphosis deformity if untreated and the chages of vertebral column axis that challanges to normal anatomy. (Figure 2
