Neonatal periungual hyperpigmentation is a clinical entity that usually affects dark-skinned neonates and it is classified among neonatal transient dermatoses. This condition is rarely seen in fair-skinned neonates. A 2-year-old boy presented with periungual hyperpigmentation of the distal phalanges of both hands and feet. Any other cutaneous or extra cutaneous abnormality was not detected. Clinical diagnosis of neonatal periungual hyperpigmentation was established. We report this case to emphasize that this clinical entity, which is rare in fair-skinned neonates, is a benign, asymptomatic and transient condition.\u
