In a clinical setting a standardized regimewas used on 33 patients with chronic or acute wounds todetect and treat a wound infection. A GC-IMS was usedas a new method for Infection Diagnostics. Wound size,germ colonization and clinical staging were documented onthree examination dates in a distance from one week. Fortreatment of the wounds a novel wound powder Toxapreventskin® ("Klinoptilolith") was used.Systematic trends were noticeable compared to the initialfindings, so that an antimicrobial effect of the treatment canbe assumed.Due to the variability of germ infestation and the smallnumber of patients assigned, a statistically significantstatement is not to be made.The results of microbiological studies using a classic woundsmear and the MCC-IMS technique show that the latterhas a distinct advantage over traditional microbiology.This methodology allows for immediate germ assignmentand can be repeated as often as necessary. An advantageof germ identification using IMS may be that the methodonly responds to clinically relevant germ growth, whiletraditional studies from the smear can also be positivefor randomly present ambient germs. The possibility ofimmediate germ assignment makes promptly specifictherapy possible.A prerequisite for a clinical application would be validationon large groups in order to define the germ-specificfingerprints even in the case of mixed infections.Based on the study results so far, it can be concluded that theToxaprevent Skin powder® has a positive therapeutic effecton wound healing, has good local tolerability and has a lowside effect profile.In order to find out further therapeutic effects principlesof the Toxaprevent skin Powder®, further clinicalapplication studies under outpatient conditions would berecommended, with the need to review local and systemicexamination parameters (e.g. local Wound assessment, germspectrum, systemic inflammatory parameters, immunology,oxygen partial pressure, proteins, muscle enzymes).
