Background: The number of hemifacial spasm patients tends to increase every year. Healing techniques and treatments such as microsurgery have also been developed. However, it is necessary to evaluate properly and accurately so that appropriate medical treatment can be chosen.Objective: This research aimed to evaluated the diagnostic accuracy of MRI 3D T2 DRIVE HR of hemifacical spasm patient which is caused by neurovascular compression compared to the result of MVD surgery as gold standard.Material and Methods: Using cross-sectional approach of secondary data and the analysis of hemifacial spasm diagnostic base don MRI 3D T2 DRIVE compared to the result of MVD surgery as gold standard at Radiology Departement of Husada Utama Surabaya Hospital.Result: The result of the research indicated the vascular that mostly causes compression toward cranial nerve VII based on MRI 3D T2 DRIVE HR is AICA (72%) and the fewest cause is SCA (3%). The result of MVD surgery points out the most common cause is AICA (75%) and the fewest cause is SCA (2%).Conclusion: The result of MRI 3D T2 DRIVE HR examination and the result of MVD surgery to decide neurovacular compression of hemifacical spasm patients indicated compability
