Pulmonary tuberculosis in childhood is difficult diagnosis taking into account the difficulty of microbiological confirmation, being diagnosis based on the presence of clinical signs, radiological alterations, history of contact with a bacilliferous adult, and interpretation of the tuberculin test. It is through the combination of these findings that the scoring systems as well as the criteria are established by the Ministry of Health, updated in 2018, since it is the score that presents the highest number of validation studies with consistent sensitivities and specificity. The same criteria are adopted by the Brazilian Society of Pneumology and Tisiology. To elaborate the article, a non-systematic review of the scientific literature was done, using the descriptors “tuberculosis”, “pulmonary tuberculosis”, “tuberculosis latent”, “child”, and “diagnosis” combined and associated with others to search and retrieve results of interest during the period from December 2017 to March 2019. It aims to carry out a review of the literature focusing on the diagnosis of childhood tuberculosis, emphasizing the updating of the scoring criteria proposed in 2018 in order to expand the segment of this risk group. Thus, faced with all sort of difficulties in the diagnosis of pulmonary tuberculosis in childhood, that if left untreated, will become transmitting bacilliferous adults, we conclude that the scoring system recommended by the Ministry of Health is an important resource for the diagnosis of tuberculosis in the validated and feasible use in any Basic Health Units (UBS).
