Intramedullary tumors are rare in childhood, with astrocytomas being more than half of the cases, presenting as a chronic disease due to indolent behavior. The report consists of a 2-year and 3-month-old child with torticollis and motor deficit in the right upper limb, which revealed an extensive low-grade glioma intramedullary tumor. After surgical resection with residual tumor, there were adverse effects to the first-line chemotherapy protocol, carboplatin and vincristine, with weekly vinblastine being instituted as an alternative. There was significant clinical improvement despite the radiological increase of the lesion.
