SUMMARY Screening and estimation of disease prevalence both often involve the use of an imperfect instrument for identifying those likely to be suffering from the disease. Sometimes an instrument designed with one of these aims in mind is used for the other. However, the aims are different, and what might be good for one need not be so for the other. The mathematics of the difference is explored and examples are given.
