In parallel with development of monitoring systems there appear more and more data concerning the efficiency of various monitoring methods and the value of studied hemodynamic parameters. Various approaches to goal-oriented therapy are being implemented and the urgency of previously proposed methods of perioperative optimization of circulation is put in doubt. There is no consensus regarding precision, clinical effectiveness and reproducibility of hemodynamic variables obtained during the perioperative period. This paper presents a contemporary view on the challenges of goal-oriented therapy in complex surgical interventions.
