Summary We present the anaesthetic management of a 66‐yr‐old male with a severely compromised airway. We describe successful utilisation of the Transnasal Humidified Rapid‐Insufflation Ventilatory Exchange (THRIVE) technique for pre‐oxygenation and provision of apnoeic oxygenation during anaesthetic induction. This technique extends the safe apnoeic period, reducing the risk of oxygen desaturation and allowing time for management of the airway.
